I forgot how heinous D&D can be that way. None of my other gms would ever start a combat with "Hey, make a save. Oops, you missed. You're dead now." On the other hand, none of the other systems I play in let dead people get up and walk around again by the time the run's over, so I guess it's ok. I've definitely seen (and made) people do the "save or be unconcious/useless for the fight" thing. "Dead" just sounds so much scarier than "unconscious," though.
Cheeses of Nazareth! Because remember, God is grated, and God is Gouda!"

There. Now you need never subject yourself to Meshuggah-Nuns. To be fair, the performers were fairly good, but the material was old when Vaudville was young. And the second act (including the above quote) was better and shorter than the first, which was good, because I was planning to walk out at the end of the first, until France showed up to join me. Trying, and failing, to live four out of ten days of game.
